Vaccination would curb Haitian cholera

Vaccination address the recent cholera epidemic in Haiti would help US researchers say. You believe the measure provided in addition to efforts to clean drinking water and improved wastewater treatment should be.

No vaccine is currently planned. Experts disagree on whether vaccination against cholera during an outbreak is effective. There is also a worldwide shortage of vaccine.

But with a mathematical model, epidemiologist Dennis Chao the Fred Hutchinson Cancer Center in Seattle, Washington, and his colleagues found, which, combined with basic public health measures that reduce the number of cases of cholera by 11% to 5% of Haitians at the top of the disease would vaccination. "I don't think there's excuse not to vaccinate high-risk individuals," Chao's co author IRA adds Longoni, an epidemiologist at the University of Washington in Seattle. But even vaccination would require 5% of the Haitian more cans - a million - than are currently available.

"This epidemic and others around the world, we recommend a comprehensive global plan for the storage and use a oral cholera vaccine to develop," says Longoni.

Haiti's first cholera epidemic in a century began in October last year. As of April 4, 274,418 Haitian was ill and 4,787 fallen after the Pan American Health Organization (PAHO), who had died the regional arm of the World Health Organization (WHO) based in Washington DC.

Nationwide, the rate of new cholera cases has fallen, but some areas see increases. This could show a second wave of the disease, says Chao.

The figure is plausible, says David sack, a vaccine expert at the Johns Hopkins University in Baltimore, Maryland, pointing out that previous epidemics have been far larger than originally predicted. "Cholera surprised us holds," he says.

But even if agreed is that vaccination would be effective, there are not enough doses in the world for the treatment of all Haitians 10 million. Last December a decision decided not to vaccinate Haiti's Ministry of health, that it is through.

"To the beginning of the epidemic, vaccination was considered, but given the limited global vaccine was care and other issues [such as fairness], cholera vaccination not as practical," Carolina Danovaro says consultant at the PAHO, a regional immunization.

There are probably between 400,000 and 600,000 doses of oral cholera vaccine around the world, says Longoni. A course of vaccination requires two doses.

One of the two vaccines available, DUKORAL, is expensive and difficult to produce, but is approved by the WHO. The second, Shanchol, is a lot cheaper. But it is still licensed in the WHO in India the approval process (see cholera tightens grip on Haiti).

The Haitian Government could use Shanchol, without waiting for the final registration, sack says. "This vaccine is safe, cheap, dosed orally and without side effects."

The International Vaccine Institute in Seoul prepares a report for the WHO, which is explained, how many cholera vaccines should be stored.

"UN had ordered the production of vaccines back in October, it would have been enough for use in areas at risk," says sack. "Not it would have a problem of limited range."

Ideally, says Chao, should keep to immunize enough vaccines to 50-70% of the population in a country with high risk for a heap. This could be deaths by half cut. Emergency vaccination plans should be coordinated efforts with rehydration and storage of rehydration kits, Chao added.

In December last year, a Committee of experts convened by the WHO recommended a small pilot study with 250,000 300,000 available doses of DUKORAL promoted. But this was not started, because the Haitian Government to these numbers small.
